The first 72 hours after a hospital stay are the most critical - and the most overwhelming. This checklist walks you through everything that needs to happen, from before you leave the hospital to the end of the first week at home.
Before You Leave the Hospital

Have a nurse or pharmacist walk through every medication
Get all follow-up appointments scheduled and written down
Ask: "What symptoms should bring us back to the ER?"
Ask if a home health referral has been ordered
Speak with the hospital case manager or social worker
Pick up new prescriptions before leaving
Arrange transportation that works for any mobility limitations
Prepare the Home

Set up a sleeping area on the main floor if stairs are a concern
Arrange any needed equipment - shower chair, grab bars, walker, or hospital bed
Stock the kitchen with easy, nutritious foods
Set up a dedicated medication station with the schedule posted
Post emergency contacts where they are easy to see
First 24 Hours

Give the first round of medications on schedule
Check the wound or surgical site as instructed
Confirm the home health nurse visit time if one was ordered
Make sure they can reach you or another caregiver easily
First Week

Track and log any changes in symptoms, appetite, or energy
Confirm ongoing home health visits and who is coordinating care
Honestly assess whether daily support is needed going forward
Contact insurance to understand coverage for any ongoing care needs
